What are these programs?
NHTD (Nursing Home Transition and Diversion) and TBI (Traumatic Brain Injury) are Medicaid waiver programs that fund a wide range of in-home services and supports — so qualifying individuals can live independently in the community instead of in a nursing facility.

Who qualifies
NHTD
Medicaid-eligible adults with a physical disability or seniors who need nursing-home-level care but want to live at home or in the community.
TBI
Medicaid-eligible individuals between 18 and 64 with a documented traumatic brain injury who need long-term support.
