Eligibility Requirements for NHTD Program

Individuals between the ages of 18 and 64 who have a physical disability or chronic illness.
Individuals aged 65 years or older.
Qualified for the level of care typically provided in a nursing home.
Possess active Medicaid coverage or meet the eligibility criteria for Medicaid coverage specifically designed for long-term care services within the community.
Additionally, the individual must demonstrate the capability to reside safely within the community, utilizing the services and support provided through Medicaid, waiver programs, and other available community resources.
