NHTD & TBI Waiver Programs

Helping people with disabilities and traumatic brain injuries leave nursing facilities — or avoid them entirely — by getting the right support at home.

What are these programs?

NHTD (Nursing Home Transition and Diversion) and TBI (Traumatic Brain Injury) are Medicaid waiver programs that fund a wide range of in-home services and supports — so qualifying individuals can live independently in the community instead of in a nursing facility.

We help families navigate the application, find the right services, and put a care team in place.

Services covered may include

  • Service coordination
  • Home and community support
  • Independent living skills training
  • Environmental modifications
  • Respite care
  • Assistive technology

Who qualifies

NHTD

Medicaid-eligible adults with a physical disability or seniors who need nursing-home-level care but want to live at home or in the community.

TBI

Medicaid-eligible individuals between 18 and 64 with a documented traumatic brain injury who need long-term support.
